Request for Ontario Student Transcript (OST)
The Ontario Student Transcript Request (OST) is the official record of a student’s academic achievement at the secondary school level in Ontario. Transcripts are required for post-secondary institutions and other reasons. If you are a current student or the last secondary school you attended was St. Nicholas Adult High School, please click on the link below and fill out the online Request for Transcript Form
